Healthcare in Andorra: CASS, compulsory insurance and supplementary cover

The short answer

Health cover in Andorra rests on local social security (CASS) for employees and self-employed people, and on private insurance for those who do not contribute to it. Valid cover is required for residency. The level of reimbursement is not equivalent to that of the French system.

The three cases

SituationCover
EmployeeCASS membership; employee contribution: 6.5 %verified on 09/10/2026, employer: 15.5 %verified on 09/10/2026
Self-employedCASS membership; contribution base: 25%, 50%, 62.5%, 75%, 100%, 125% or 137.5% of the average monthly overall wage of the previous yearverified on 12/10/2026
Resident without an activityPrivate insurance in general

Practical steps

  1. Define your cover before filing the residency application.
  2. Obtain the required certificate. See documents.
  3. Choose a doctor and a reference hospital.
  4. Take out supplementary insurance suited to your age and your family.

What the law requires in order to reside

For residencies without a gainful, professional, scientific, cultural or sporting activity, and for remote workers, the applicant must prove sickness, incapacity and old-age cover valid in Andorra for themselves and their dependants, for the whole duration of the permit. For minors and people over sixty, sickness cover is enough. See documents, timelines and costs.

What the CASS covers (CLEISS sheet, 2024 data)

BenefitRule
Outpatient care65% or 75%verified on 12/10/2026 of the reference tariff
Hospitalisation90 %verified on 12/10/2026 of the reference tariff
Sickness: daily allowancesWaiting period of 3 daysverified on 12/10/2026, then 53 %verified on 12/10/2026 then 66 %verified on 12/10/2026 of salary
Maternity20 weeksverified on 12/10/2026 at full salary
Paternity28 daysverified on 12/10/2026 consecutive

Reimbursement rates are calculated on a reference tariff, and a professional with no agreement with the CASS is reimbursed at a lower rate: supplementary insurance is therefore common.

Retirement

The statutory age is 65 yearsverified on 09/10/2026 with 180 monthsverified on 09/10/2026 of contributions, and early retirement is possible from 61 yearsverified on 09/10/2026 with 480 monthsverified on 09/10/2026, with a reduction of 7 %verified on 09/10/2026 for each year of anticipation: see retirement age.

Family benefits

They require legal, effective and permanent residence of at least 7 yearsverified on 12/10/2026, with income ceilings. A newcomer is therefore not entitled to them straight away: allow for this in the family budget.

To be confirmed

The rates come from a 2024 sheet: confirm them with the CASS. See also social security for cross-border workers.
